Physical Examinations
Comprehensive
physical exams are required for all new students. They are also
required for students in kindergarten and in grades 2, 5, 8 and 11. Additionally, students participating in interscholastic sports (grade 7-12) must have a
physical
on file every thirteen months to the day of the exam. For example, if a
student-athlete has a physical exam dated September 14, 2011 that exam
will expire on October 14, 2012. Meaning as of October 15, 2012 the
student athlete becomes inelgible to practice or compete until there is
proof of a current physical. No doctor's notes will be accepted to
bridge the gap between physicals. There will be no exceptions. You may obtain a Physical Exam form from the school nurse or you may download the form below.

If
you do not have a local health care provider, please see the school
nurse who can provide you with a list of nearby medical offices and
clinics. If you do not have health insurance, the nurse has information
about Massachusetts state health insurance programs and reduced
-fee/free clinics.
